The only time I ever broke my Oath of Vengeance was when I 'accepted' Gortash's deal during the coronation, and when I allowed Astarion to ascend, both times I could argue aren't that bad. I've yet to play the other 2 Oaths, and after Astarion ascended, I stuck with my broken Oath, so there are a few final encounters I am unsure of.
